● How well do they know where the individual is at all times like for example whether they are using their walking devices so they don't fall or attending social activities? ie staffing ratio, staff culture, cameras/monitors

● How much do they encourage/support individuals to do group social activities? ie do they bring them out to a central dining room, the physical setting feels like a home, not hospital

● Cleanliness? Is there odor of urine/stool in the hallway, age of facility, cleaning schedule, how cleanliness monitored

● How easy is it to reach someone?
